Please note as of the 1st of April when collecting prescriptions from the health centre you will be asked to provide your name, date of birth and full address. This is to ensure that prescriptions are given to the correct people and to protect patient's confidentiality. If you wish for someone else to collect your prescription then they will be asked for these details and asked to sign a form to say they have collected it. If they cannot provide this information they will not be given the prescription.
